Why AI Makes Things Up (And What to Do About It)
AI systems work by predicting. Specifically, they predict what words and sentences should come next based on patterns in text they were trained on. They're extraordinarily good at this. The responses are fluent, confident, and often correct.
The problem is that this same mechanism produces wrong answers in exactly the same tone. The system doesn't know when it doesn't know something. It just keeps generating. If you ask it a question it has incomplete information about, it will give you a plausible-sounding answer that may be completely fabricated.
This has a name: hallucination. It's a real phenomenon in AI systems. It's also widely misunderstood. Most people think it's a bug that engineers are working on fixing. It's not. It's an inherent characteristic of how large language models work. It improves with better models and better training, but it doesn't disappear. The underlying mechanism guarantees it will happen.
Before you use any AI tool — at work or at home:
Never enter your employer's sensitive information into a free AI tool. This includes customer names or contact information, employee records, financial data, contracts, proprietary processes, or anything you'd consider confidential. Free AI tools are public services. Treat them accordingly. If you're not sure whether something is safe to enter, don't enter it. Ask your manager first.
Here's a manufacturing analogy that makes the distinction clear.
Imagine a quality technician who has worked on your line for twenty years. They know the process cold. Ask them about a specification they've handled a thousand times and their answer is reliable. They'll tell you the torque spec, the material grade, the acceptable tolerance range. They know because they've done it so many times the answer is automatic.
Now ask them about a specification from a product line they've never touched. A good tech will say "I'm not sure, let me check the work instruction." They know the limits of their knowledge and they defer to documentation.
Now imagine a tech who always gives a confident answer, regardless of whether they actually know. Specification they know cold? Confident, correct answer. Specification from a product line they've never touched? Still confident. But the answer is whatever sounds plausible based on general knowledge of specs. Sometimes right. Sometimes completely wrong. But delivered with the same certainty either way.
That hallucinating tech is like a language model. The model can't distinguish between "I was trained on thousands of examples of this" and "I've never seen this before." It generates a response either way.
What to do about it comes down to one rule: never use AI output for anything where being wrong has a real consequence without verifying it yourself.
Specific examples from manufacturing: don't use AI to interpret a specification sheet and act on the numbers without checking the actual document. The AI might have been trained on similar specs and make a plausible guess that's actually wrong. Don't ask AI a safety question and follow the answer without checking the actual procedure. The stakes are too high. Don't let AI write a work instruction that nobody with floor experience has reviewed. The instruction might sound official and reasonable but miss critical details or be genuinely wrong on the specifics.
The places where AI is genuinely useful: generating a first draft that you then review and correct. Summarizing long documents so you can spot-check the important parts instead of reading everything. Explaining a concept in multiple ways so something clicks. Organizing information so you can evaluate it more easily.
In all those cases, you're the decision maker. The AI is generating options or handling volume. You're verifying.
Here's why this matters operationally: the more people in your organization who understand that AI hallucination is not a minor bug but a core feature, the less likely bad decisions get made. Someone uses an AI system to get a quick answer about something critical. They verify the answer. The answer is wrong. They catch it. That's good. It's also why using company-approved AI systems with proper training matters more than just using free tools. A company that's doing AI right is training people on what these systems can and can't do. A company that just lets employees loose with free ChatGPT is hoping for the best.
The tool is useful. But it's useful in the same way a reference book is useful — it's a starting point. Not an authority.
